Position: Community Health Worker- Part time 32 hours/week
Shift: Monday- Thursday- 8am-4:30pm- LMC home site- will travel to community partners based in West Philadelphia.

Experience:
Minimum 2-years' work or volunteer experience in human services or healthcare.

Education:
High School diploma or equivalent required

Other Requirements: With limited exceptions approved by the supervisor in advance, a CHW is expected to have a valid U.S. Motor Vehicle License and reliable transportation to travel to/from outreach activities, as they may be assigned from time to time. Demonstrated technical experience with Microsoft Word, Excel (or similar databases), and Internet Explorer
