Responsibilities

The Clinical Pharmacist - ID Specialist supports pharmacy services through active evaluation of antimicrobial agent use and coordination of antimicrobial stewardship activities for designated hospital(s). The Clinical Pharmacist - ID Specialist will work closely with other members of the local pharmacy management team, Infectious Diseases physicians, infection control team, and the local/regional Antimicrobial Stewardship Committee to optimize antimicrobial use and de-escalation. This position will act as a medication resource and provide high quality care for patients receiving antimicrobial therapy through individualized clinical patient evaluation, targeted drug evaluation, assessment of susceptibility trending data, evaluation of formulary drug use, as well as cost-benefit evaluation. Primary responsibilities include safe and optimized antimicrobial use for patients; development and coordination of antimicrobial stewardship activities; participation in multidisciplinary care rounding with ID physicians; order verification; clinical intervention; teaching/precepting hospital staff, medical residents, pharmacy students, and pharmacy residents; and participating in appropriate local and system antimicrobial stewardship projects and committees.



Qualifications

EDUCATION, EXPERIENCE, TRAINING

Required qualifications:

1. Current state Registered Pharmacist License.
2. B.S. in Pharmacy or PharmD degree required.

3. PGY-1 Pharmacy Practice Residency required (significant experience may be considered in lieu of residency)

4. Minimum of:

a. 3 years of significant direct patient care experience in infectious diseases; OR

b. PGY-2 Pharmacy Practice Residency and/or fellowship training in the area of Infectious Diseases

5. Strong verbal and written communication skills required.

Preferred qualification:

1. Board Certification in Infectious Diseases (BCIDP) preferred.

2. Experience as a pharmacy residency preceptor in the area of Infectious Diseases preferred.
