Case Manager, Residential Treatment Center

Responsibilities

Job Summary
Our Residential Treatment Center holds two 12-bed units and one 10-bed unit for youth ages 10 - 18 who require long-term treatment for severe and chronic psychiatric disorders. We are dedicated to working with our residents and their families to develop specific behavioral plans to help patients successfully transition to a less restrictive therapeutic environment.

This position will lead a multidisciplinary treatment team in developing a treatment plan for each patient and document information in progress notes. Working closely with the Clinical team within the Residential Treatment Center, this position will be responisble for ensuring the proper procedures are followed to effectively assist the residents.

Responsibilities

  • Provides individual therapy according to treatment plan.
  • Provides group counseling sessions.
  • Assures coverage of scheduled activities such as educational and therapeutic groups.
  • Coordinates the development of a therapeutic milieu through an honest, supportive and structured atmosphere.
  • Keeps informed of community treatment resources from referrals to and from the program for patients and their families as evidenced by listing service for mental health, group home, assistance programs, 12-step programs and DCFS.
  • Participates in program planning (recommends curriculum, program changes, etc) with other staff and supervisor.
  • Develops referral resources and maintains regular contact

Skills

  • Behavioral Health experience
  • Strong organizational skills to manage multiple cases simultaneously while maintaining attention to detail.

  • 8 hour shift

Qualifications

Education:

  • Bachelor's degree in the human services field (Required)

Experience:

  • Previous professional experience in a behavioral health setting preferred.
