Position Description

NCI Division, Peoria Area Command

Position Title: Pathway of Hope Case Manager

Job Summary: Provide strength-based case management for individuals/ families who are seeking to participate in the Pathway of Hope program targeted to break the cycle of crisis and enable a path out of intergenerational poverty.

Reports To: Primarily reports to the Peoria Area Social Services Director and works in coordination with the local and Divisional Pathway of Hope program staff.

Qualifications: A minimum of two years of progressive years of case management and client support in the social services profession or related educational experiences. Individual must be able to work in a confidential manner and manage the varied demands of working with vulnerable, high-risk individuals and families.

Candidates must have a passion for fulfilling the mission of The Salvation Army and must be articulate in presenting the same to others.

Candidates must successfully pass the pre-employment background inquiry and annual motor vehicle check with a valid driver's license.

Education: Bachelor's degree in social work sociology, criminal justice or other related field preferred; significant related experience may be considered in lieu of a degree.

  • Ability to efficiently and effectively manage people and projects successfully.
  • Team player, self-starter, and perform well with minimal supervision.
  • Must have an interest and ability to work with people of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
  • Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member.
  • Proficiency in computer software applications, including demonstrated experience in using email, MS Office applications, and SIMS.
  • Must be able to work days, evenings, weekends and holidays as necessary.

Essential Responsibilities:



  • Conduct outreach and engagement services in the community in effort to identify and support participants.
  • Engage and build rapport with the target population and meet the desired outcomes of eligibility requirements.
  • Collaborate with local leadership to best support clients and their designated goals.
  • Provide appropriate referrals and linkage to support individuals and families of the local community.
  • Perform client intake including any assessments and measurements and ongoing follow up needs as appropriate. Must be able to assess any serious personal safety and mental health issues. Provide information on available community resources and referrals as necessary. Maintain an accurate and updated case plan to support stability and self-sufficiency to include both short and long-term goals with related activities and action steps.
  • Work in coordination with Women and Family Shelter Case Manager to provide a continuation of services to clients moving out of Peoria Services Women and Family Shelter into housing in the community to achieve self-sufficiency and permanency
  • Maintain written and electronic case files with all relevant client information along with advocacy and financial services as necessary. (i.e., court appointments, landlord/tenant conflict resolution, etc.)
  • Engage and collaborate by participating in monthly supervision, divisional case manager call-ins and local Pathway of Hope team meetings driven by the POH Regional Coordinator to ensure best practices.
  • Must be able to enter in all data required into the Salvation Army Information Management system promptly with a high level of accuracy.
  • Attend conferences, seminars and training as directed including regular staff meetings as hosted.
  • Case Coordinate with ICC students enrolled in higher education programs.
  • Maintain working relationships with community agencies to provide comprehensive services for participants and to stay abreast of current trends and resources available.
  • Maintain positive work atmosphere through appropriate conflict management, courteous demeanor, and professional and appropriate communication practices
  • Maintain compliance in The Salvation Army's "Safe from Harm" Certification and other designated trainings.
  • Other duties as assigned by the Director.


Position Ranking:



  • Regular Full Time, Non-Exempt


Physical Demands/ Work Environment:

This Position is required to do basic business and office work. To successfully perform the essential functions of this position, the individual is regularly required to talk and listen, stand, walk, sit and use hands. Individual must be able to regularly lift to 30 pounds and occasionally lift to fifty pounds. The use of Office and Computer Equipment is necessary. Vision abilities will require close vision. Work may be performed indoors and occasionally require outdoors to attend meetings. There is low to moderate noise level for this position.
