MDS Coordinator -Full time

Registered Nurse

The MDS Coordinator is responsible for assuring the timely completion of accurate assessments and interdisciplinary individualized care plans that meet Federal and State guidelines.



  • Works in collaboration with the Interdisciplinary Team to assess the needs of the residents and then completing the MDS &CAAs.
  • Works with the Interdisciplinary Team in setting the Assessment Reference Dates. Will assist in identifying residents in need of a Significant Change of Status Assessment.
  • Supports the tracking system of MDS schedules to facilitate the timely completion of MDS sections and CAA's by each discipline (timeframes and due dates).
  • Ensures documentation is present in the medical record to support MDS coding.
  • Evaluates outstanding MDS's for transmission readiness monitoring logical sequencing and submission timeliness.
  • Assists in completes timely electronic transmissions of MDS's to the state database and reviews validation report correcting fatal errors immediately and retransmitting the assessment, addresses non-fatal errors and maintains validation reports.
  • Assists in the oversight of the Restorative Nursing Program ensuring proper documentation and providing staff education.
  • Participates in department and facility CQI teams as goals and needs require.
  • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members
  • Current Licensed Wisconsin Registered Nurse
  • Knowledge and experience with MDS completion and Nursing Supervision.
  • Two years geriatric nursing experience or equivalent with sound background and knowledge regarding Medicare and MDS guidelines.
  • Maintains knowledge of current nursing practice and regulations through attendance at seminars, in-services, etc.
  • Knowledge of HFS 132 regulations.
  • Works with the realization that errors may have serious consequences for residents and coworkers.
  • Actively participates in or initiates problem solving for job-related issues.
  • The ability to speak to and hear residents and staff; also to read, write, and communicate using the English language.
  • The ability to deal diplomatically and effectively with residents, staff, family members, physicians, visitors, and volunteers.
  • Good communication, organizational and leadership skills with attention to detail and accuracy.
  • The mobility to spend 50% of work shift moving about the facility.
  • The mobility to respond quickly in emergency situations in all areas of Luther Manor campus.
  • The physical strength to move or assist residents as required in emergency situations following their plan of care in all areas of the Luther Manor Campus.
  • Personal computer skills include use of integrated long-term care system.
