Inova Fairfax Hospital is seeking a dedicated Solid Organ Transplant Clinical Pharmacy Specialist to join the team. This role will be full-time day shift from Monday - Friday | Sign-on Bonus & relocation assistance eligible.

Solid Organ Transplant Clinical Pharmacy Specialist Job Responsibilities:



  • Facilitates the development and implementation of treatment guidelines, protocols, formulary changes and critical pathways.
  • Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff.
  • Provides clinical consultation and clarification to practitioners and patients and suggests appropriate and cost-effective therapeutic alternatives to medical staff as needed.
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions and appropriateness of drug/dose.
  • Prepares and presents drug reviews, MUE criteria, antibiotic sensitivity reports, adverse drug reaction reports and pharmacy intervention summaries to the Pharmacy and Therapeutics Committee.
  • Works under a collaborative practice agreement with provider team.


Solid Organ Transplant Additional Requirements:



  • Work schedule: 8-hour shift (Monday - Friday day shift).
  • Education: Doctorate of Pharmacy (PharmD).
  • Experience: Five (5) years of solid organ transplant experience or PGY1/PGY2 solid organ transplant residency required.
  • Certification: Licensed Pharmacist in the state of VA.
