POSITION SUMMARY: The LTSS Resource Coordinator (RC) will dedicate 100% of their time providing clerical/admin support to the Care Coordination staff in the provision of LTSS. The RC's work will emphasize quality of service delivery and Enrollee advocacy, especially in securing needed resources and services in timely fashion. Position: Resource Coordinator Department: LTSS- BAP Program Schedule: Full Time ESSENTIAL RESPONSIBILITIES / DUTIES:
Perform Enrollee and Caregiver outreach calls as directed by Clinical Care Manager and/or Care Coordinators Assist LTSS Care Coordinators in the completion and management of relevant applications and other forms required for securing resources for Enrollees and their caregiver(s), including Enrollees with special needs Support communications between the LTSS Care Coordination staff and relevant community-based and state agencies in securing IL-related resources and services for Enrollees Follow up with relevant parties to close the loop on open applications for services and resources (i.e., verify approval for and start of services), documenting all findings in the electronic health record Perform other tasks, as required

JOB REQUIREMENTS Education:
Bachelor's Degree in Social Work, Psychology, Education, Public Health, or a related field required. Master's Degree preferred.
Experience:
At least two (2) years' experience with patient advocacy or community-based care coordination, with a focus on specialized populations (e.g., patients with physical and/or intellectual disabilities) Two (2) years of experience working with individuals with complex medical/behavioral and/or psychosocial needs

Strong oral and written communication skills Bilingual strongly desired (e.g., Cape Verdean Creole) Ability to establish and maintain effective working relationships with Enrollees, social service agencies, community resources, and other members of the interdisciplinary team. Ability to work with Enrollees to promote positive behavioral change Strong Motivational Interviewing skills Strong organizational and time management skills Ability to work in a fast-paced environment and multi-task Proficient Microsoft Office skills particularly MS Outlook and MS Word and other data entry processing applications Knowledge of medical terminology
Working Conditions and Physical Effort:
