DaVita is hiring a Divisional Lead Social Worker to lead our Social Work Teams in the Surf N Sun and WestSide divisions of Dream Team. Candidates must live in one of the two divisions (Arizona, Northwestern New Mexico or the following locations in CA - Riverside, San Bernadino, and parts of Los Angeles Counties). Dialysis experience is required.

Responsibilities

  • The Divisional Lead Social Worker ("DLSW") is responsible for the delivery of DaVita's Social Work model of care in two designated Divisions.

  • The DLSW is an essential communication link and professional resource for the Social Workers and Operations, about 125 MSWs in the two divisions.

  • The DLSW is a mentor and coach as well as a knowledgeable resource.

  • This DLSW will not have direct patient care responsibilities and will be 100% remote, with travel to meetings in both divisions as well as nationally required throughout the year.

  • The DLSW will directly report to the Sr. Manager Social Work Operations.

  • Quarterly travel to divisions supported, 3-5 days per quarter.

  • Annual travel to HQ 2 times per year.

Divisional Lead Social Worker Essential Duties

  • Oversees RLSWs and MSWs within the Division. Conducts 1:1 and/or group operational reviews with RLSWs monthly to promote divisional success. Serves as leader and mentor to RLSWs, field social workers.

  • Supports education and training within Division. Communicates vision for Social Work, Village, Division, and Department's goals to MSWs and other leaders within Division.

  • Leads implementation of new social work initiatives within Division.

  • Works collaboratively with the National Social Work team to evolve and implement the Social Work model of care and ensure compliance with policies and procedures.

  • Collaborates with DCS and DVP to align and prioritize social work activities with Village and Division goals, and address social worker-related issues within the Division.

  • Participates as a member of the national DLSW team, and on other Village-wide Social Work committees as needed. Represents Social Work on other cross-functional committees, in partnership with Divisional Team (includes DCS, DLRD)

Division Lead Social Worker Job Requirements

  • Master's Degree in Social Work from a school accredited by the Council on Social Work Education Required Preferred

  • MSW with 24 months of specialized renal experience required

  • Schedule flexibility and ability to travel required

  • Interest/willingness to contribute to excellence for Social Work required

  • Intermediate computer skills; Proficiency with Microsoft Office and DaVita applications required

  • Licensed or registered in either Arizona, California, or New Mexico as required in the state of practice required

  • LCSW or equivalent clinical license even if not required by state preferred
