DaVita is looking for an NP Lead. The Lead, Nurse Practitioner ("NP Lead") functions autonomously, yet in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESKD and/or CKD patients. The NP Lead will focus on improving clinical outcomes, care delivery, goal setting, and onboarding new NPs. The NP Lead will join us in the AR, AL, and W. TN market. Position Description: NP Lead will train and mentor Nurse Practitioners, Support Nurse Practitioners to complete CHEs (Comprehensive Health Evaluations). When seeing patients, complete comprehensive office visits on the go- primary care focused visits (chair-side in dialysis clinic, in-person visits and telehealth). See and manage complex patients in a short amount of time- 30 minutes. Location: Round and Travel within an assigned geography to support NPs (trainings, meetings, and shadowing) and patients as needed. As the NP Lead you will perform Comprehensive Health Evaluations during patient visits in dialysis clinics, hospitals, when necessary to provide service to identify and assist with patients' needs including but not limited to medical, social, emotional and financial concerns through medical record review, consultation with patient, family and healthcare team members. As the lead you will act as the clinical lead for a team of NPs; assist with onboarding new NP's; complete ongoing training for NPs through clinical best practice exercisesand provide ongoing support to other NPs.
