Job Functions:

This position leads the team that is responsible for establishing and cultivating community events and health programming to families in need in Hillsborough and Pasco counties. Strategize, coordinate, and communicate with program leadership, volunteer services, food services, staff, and other internal departments who facilitate client health programming. This position offers a flexible work schedule with a minimum of 3 days of physical reporting to the office, including occasional travel to support Pasco program. This role may also require additional days of in-office reporting, weekend support, and duties that exceed normal workday hours.

Essential Responsibilities:

Oversees the following activities in both Hillsborough and Pasco:

  1. According to the mission and vision of the organization, leads the development and execution of prevention strategies that impact positive health outcomes, lower health costs, and increase community engagement for families in need.
  2. Hires, trains and supervises all Health and Wellness Navigator staff in Hillsborough and Pasco and leads planning for future growth in additional counties.
  3. Implement and maintain innovative health and wellness programming designed to encourage healthy lifestyles, increase family health education and improve family health outcomes, including weekly Health and Wellness activities.
  4. Works with clients to develop and maintain Health and Wellness goals. Creates objectives for their programs and tracks progress. Engages and supports families as they try to increase their health and wellness outcomes.
  5. Assess nutritional needs of clients and coordinates with kitchen, outreach, and in-kind departments to ensure nutritional needs are met. Education and support to clients on nutritional elements. Works with kitchen staff to ensure client dietary needs are accounted for.
  6. Monitors Health and Wellness policies and procedures. Keeps policies and procedures up to date with growing Health and Wellness needs. Consult with VP for policy change approval. Works closely with HR on Health and Wellness support for staff.
  7. Seeks and manages grants and funding opportunities for the health and wellness program/services; ensures grant outcomes are met; provides groups, documentation, tracking and monthly updates to ensure annual goals are met. Strategizes on modifications if goal expectations are not met and need revision.
  8. Represents Metropolitan Ministries at strategic and organization supported community events, network meetings and pursue partnerships that will enhance/support services and community involvement as determined by organization strategy. Provides tours of the campus facilities.
  9. Acts as liaison to all medical and health and wellness partners and providers.
  10. Represents MM in activities and events related to health and wellness.
  11. Works with other departments to improve processes, meet growing needs, and involve supervisor as needed when there are barriers to reaching given competing demands that prevent workable solutions.
  12. Maintains weekly tracking of client health progress and assist clients in preparation of medical procedures (pregnancy, surgery, etc.) and making accommodations within their living quarters for recovery.
  13. Maintains real time notations related to Health and Wellness into HMIS database.
  14. Ensures completion of individual family health assessments of all families participating in residential programming. Also ensures that families are provided food bins and YMCA memberships once completed.
  15. Maintains tracking of resident Emergency Shelter visits, actively seeking opportunities to minimize the expense of ER costs through virtual doctors and telehealth.
  16. Maintains allotted Health and Wellness budget annually for H&W equipment, events, and services. All purchases will require approval by Senior Director of Residential Programs.
  17. Meets personal goals established in individual development plan, KPI's, and based upon annual evaluations and through regular supervision. Actively contributes to the achievement of the team and organizational targets/goals. Supports the team in other tasks as assigned. Adheres to all policies of Metropolitan Ministries.
Requirements

Education and Experience:

Master's Degree in the Public Health, Health Education, or similar along with minimum of 2 years' experience providing health and wellness services for at risk population. At least 2 years of Supervisor experience preferred. Must understand inter-relationship of stress and trauma impact on health/outcomes.

Skills Requirements:

Must be able to work effectively with homeless and at-risk clients. This position requires the ability to work within a multi-disciplinary framework; should be computer literate proficiency with various computer applications preferred (including, but not limited to, Microsoft Office applications) with knowledge of Microsoft Word, PowerPoint, and Excel desirable. Must be able to communicate orally, as well as in writing, and demonstrate a high level of comfort in this performance, conducting and assisting in the conducting of meetings, etc. Strong interpersonal skills are required. This position requires a high degree of maturity, confidentiality, sensitivity to issues of faith and cross cultural and diversity issues, and the social skill to handle job-related matters in a professional and diplomatic manner.

Physical Requirements:

Must have adequate hearing and speaking ability as to take part in and provide sound oral communication and information. Must be capable of working outdoors in sunlight and higher temperatures on occasion. Must have the physical, emotional, and spiritual stamina to handle job-related issues and stress. Ability to work out with clients on occasion to assist clients in creating healthy habits to meet their personal health/wellness goals and establish new health habits.

Other:

Must demonstrate legal authorization to work in the United States. Must be able to communicate effectively in English. Valid FL driver's license with a good driving record and insured vehicle in order to qualify for driving Metropolitan Ministries vehicles. Bi-lingual in Spanish a plus but not required.
