Corewell Health West Geriatrics provides team-based primary care to approximately nine hundred patients in their homes, as well as medical care in 19 local long-term care facilities and continuing care communities. Patients served are typically frail, elderly, and in need of supportive services. The Nursing Supervisor position oversees approximately thirty nurses: RN care managers in the Home-Based Primary Care program as well as LPNs who provide telephone nurse triage for both home-based and long-term care. Our ideal candidate will have long term care or home care background with evidence of supervisory experience. This RN Supervisor will have oversight of a team of approximately thirty made up of Care Managers and LPNs. Scope of work Directs the activities of clinical staff within assigned area and ensures the delivery of efficient, smooth patient flow and patient satisfaction. Essential functions
