Community Support Specialist UnityPoint Health - Black Hawk Grundy Mental Health Center Shift: Full-time; Days Benefits Eligible Potential Sign-On Bonus Eligible: $5,000 paid out over 1 year The Community Support Specialist is responsible for providing a wide variety of community based support services to individuals with a Serious Mental Illness (SMI) designed to reduce and/or prevent emergency department visits, mental health unit admissions or readmissions and placement in a more restrictive environment. Responsibilities
- Provides daily living skills training including but not limited to development of medication management strategies, shopping, learning the transportation system, symptom management skills, locating housing, coping strategies, transportation to selected appointments, etc.
- Provides a wide variety of "care coordination" or case management activities including but not limited to information and referral, education, setting up appointments, setting up transportation, working with others involved in the client's life, responding to crises, etc.
- Documentation of all services provided in BHGMHC's Electronic Health Record according to BHGMHC standards, insurance and/or managed care company requirements, state standards, etc.
Qualifications
Minimum Requirements:
- Bachelor's Degree in a Human Service-related field - Bachelor's Degree in Social Work preferred
- 1-3 years' experience in the mental health field
- Ability to form good working relationships with clients
- Flexibility
- Orientation to community-based services
