Panel Attribution Concierge Coordinator

GENERAL SUMMARY:

Under limited supervision of an administrative director, works independently to provide support managing panel attribution up to 30,000 (+/-) aligned member lives for primary care physicians (PCPs). Works with administrative director, department Chair and providers to provide panel concierge service including but not limited to pulling, analyzing, organizing, and correcting panel attribution. Collaborates with payers and other Henry Ford Health departments to assure accurate panel attribution to help ensure all value-based contracts and risk arrangement incentives are fully captured for Henry Ford Health PCPs.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

* Manages complex payer member files in conjunction with EPIC reports to reconcile payer and EPIC attribution.

* Develops and implements databases, data collection systems, data analytics, and other strategies that optimize efficiency and quality.

* Audits reports and identifies, analyzes, and interprets trends or patterns in data sets.

* Works with payers to ensure accurate member attribution to Henry Ford Health PCP for up to 30,000 (total # may fluctuate based on payer, contracts VBR, etc.) aligned lives.

* Prepares and organizes meetings, reports, presentations, and documents related to member engagement and attribution assignment for assigned providers/members.

* Performs patient outreach as directed to close quality care gaps and/or attribution and engagement with Henry Ford Health primary care providers.

* Maintains working knowledge of payer initiatives/processes, including new initiatives and updates. Participates in educating key stakeholders on above.

* Works with other business units and disciplines to assure attribution and registration processes are followed. Educates new Clinic Service Representatives and department staff on attribution and registration processes.

* Maintains registry compliance scores in accordance with network participation standards.

* Performs other related duties as assigned or requested.

EDUCATION/EXPERIENCE REQUIRED:

* Associate degree in liberal arts, business, analytics, information technology, health care related field or medical coding.

OR

* High school diploma. Medical coding certificate and one (1) year of medical coding experience.

OR

* High school diploma. Three (3) years of experience in health care business operations (e.g., payer relations, admissions, billing/coding, authorization etc.), analytics, information technology, health care related field or medical coding.

* Bachelor's degree in any field preferred.

* Must be able to work independently on assigned work and communicate with leaders when issues arise.



  • Computer software, i.e., Microsoft Office products such as Word, Excel, Power Point, Access skills required.
