We are seeking Assertive Community Treatment (ACT) Team Leader to join our Adult Behavioral Health Division in Canonsburg and Waynesburg, serving Washington and Greene Counties
Position Details We are seeking a dedicated and experienced Team Leader to oversee and direct our Assertive Community Treatment (ACT) Team. This role involves managing a self-contained clinical team responsible for providing comprehensive treatment, rehabilitation, and support services to clients with severe and persistent mental health illnesses. The Team Leader will supervise and evaluate a multidisciplinary team, ensure service excellence, and act as a practicing clinician. * Oversee daily clinical operations of the ACT Team, including coordinating staff work schedules to ensure adequate coverage. * Facilitate daily team meetings and maintain a daily log of these meetings. * Direct and coordinate the client admission process, including scheduling interviews, developing initial assessments, and coordinating treatment plans with the psychiatrist. * Oversee assessments in areas such as family structure, substance use, education, employment, social development, daily living activities, nursing, and psychiatric evaluations. * Provide clinical supervision and general oversight of team members, including weekly individual supervision and monthly field supervision. * Build and maintain relationships with staff, clients, community resources, and external human service agencies as needed. * Ensure client medical records comply with agency, Medicaid/Medicare, and other governing policies and standards, and meet all third-party payment requirements. Maintain logs for hospital and incarceration cases. * Assist with staff recruitment, interviewing, hiring, performance evaluations, and training. Enforce company policies and address employee relation issues as needed. * Review program financials as required. * Serve as second on-call for crisis situations and perform on-call duties as required. * Perform other essential job functions as needed and participate in cross-training with other specialists as deemed necessary by the supervisor. Assertive Community Treatment (ACT) ACT teams are composed of professionals including psychiatrists, nurses, mental health professionals, substance abuse professionals, employment specialists, housing specialists, and a peer specialist, all of whom collaborate to provide complete services including therapy, medication management and resource linkage to adult individuals with serious mental illness (SMI). Services are provided in the individual's home or the community and include at least three home visits a week. The ideal candidate will possess the following qualifications: * Masters' Degree in the human services field of Psychiatric Rehabilitation, or Psychology * Supervisory and program management experience is required * Professional license eligible * Strong clinical skills and experience providing treatment to persons with severe and persistent mental illnesses * Clear understanding of the characteristics and problems of adults with severe and persistent mental illnesses * Knowledgeable about the Assertive Community Treatment (ACT) model Equal Opportunity Employer Minorities/Women/Protected Veterans/Disabled |
