Schedule: 8:00am-4:30p; every other weekend
Why You should be our next Resource Specialist: You will work with Nurse Care Coordinators, Social Workers, and other care team staff to ensure that patients receive the resources and services they need to successfully return to a community setting, including home with services (i.e. Visiting Nurse Association) or without services, long term care facility, community hospital, or outpatient clinic. The Resource Specialist is responsible for assisting with the development of education and referral information regarding extended care facilities, placement process, and homecare and community resources, including levels of care, long term care assessment forms, and Medicare/Medicaid rules and regulations regarding placement. The Resource Specialist is responsible for acting as an advocate for patients and patient families and strives to support the hospital's aims for optimal resource management, high customer satisfaction, and high quality care.
