Responsibilities

Are you ready to make a difference in the lives of individuals with psychiatric needs? As a Social Worker, you'll be the go-to person for providing support and services to our amazing patients!

Responsibilities and Accountabilities:

  • Knowledge and Understanding: Have knowledge and understanding of Social Work practices and principles.
  • Assessment and Planning: Conduct effective assessments and suicide risk evaluations, develop treatment and discharge plans, and lead educational or therapeutic groups.
  • Patient and Family Communication: Maintain open lines of communication with patients and their families to ensure they are actively involved in treatment decisions.
  • Care Coordination: Coordinate care with outpatient providers during patient admissions and discharges to ensure continuity of care.
  • Team Facilitation: Facilitate weekly treatment team meetings, creating and updating master treatment plans based on team consensus.
  • Clinical Contribution: Bring a unique clinical perspective to the interdisciplinary team, drawing on training in family systems and social work theory.
  • Effective Communication: Communicate clearly and effectively with clinical team members and Directors to ensure cohesive teamwork.
  • Cultural Competence: Be knowledgeable about gender, cultural differences, and age-appropriate care to provide the best support for patients.

Qualifications

  • Bachelor's Degree in Social Work Required, with Master's Degree in Social Work preferred
  • Must have licensure in the State of Minnesota
  • Experience in healthcare setting preferred
  • Experience using electronic health records preferred
  • Proficient in Microsoft Office Products including Word and Outlook
  • Able to effectively and efficiently communicate in written and verbal form
  • Ability to smoothly and rapidly shift between dissimilar tasks
  • Ability to communicate well both verbally and in writing
  • Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
  • Ability to make effective decisions while in stressful situations and circumstances
  • After training, be able to manage disruptive and explosive patients.

Compensation for this role ranges from $24-$34 per hour depending on education and experience.

An individual's pay rate within the posted range is determined by several factors, including skills, experience, education, and market conditions.
