JOB DESCRIPTION: SUMMARY OF POSITION Assists Clinic Outpatient and Access Services staff in engaging clients in treatment services, screening and following up with high-risk clients, assisting with care coordination and life skills as needed during transition to other services, and connecting clients to necessary resources. KNOWLEDGE, SKILLS & ABILITIES Effectively communicate via written, verbal, in person and virtual methods. Knowledge of community geographical awareness and of community and agency resources Skill in customer service techniques. QUALIFICATIONS Education Level Associate degree required, Bachelor's degree preferred. Years of Experience At least two years of direct service experience working or volunteering in a human services organization is preferred.
