In this position, you will work within a multi-disciplinary health care team and report to the Director of Health Education. The Health Educator is responsible for the assessment, planning, coordination and implementation of health education programs that focus on a variety of subjects related to perinatal services, such as prenatal education, physical and emotional chances of pregnancy, reproductive health education, car seat safety, and newborn/infant care education. Other key responsibilities include:
- Case Management - Completing intake assessments, second and third trimester reassessments, and Postpartum assessments for patients. Providing education, appropriate referrals, and ensure linkage.
- Health Education Delivery & Development - Deliver maternal and child health information to prenatal and postpartum patients, coordinate and implement health education sessions and other health education presentations, and oversee and manage perinatal health education resources.
- Community Outreach - Provide information about the Clinic to community members and serve as a liaison between the Clinic and the community.
- Administration - Monitor and evaluate targeted intervention practices, contract requirements, and submit complete patient encounter forms in a timely fashion. Coordinate and manage health education services tracking and submit monthly reports to the Health Education Services Supervisor.
- Travel between clinics and community organizations to conduct health education programs, presentation, services, outreach, and other related activities.
- Participate in meetings and projects, create reports, and perform other duties as assigned by management.
Salary offers are determined based on various factors including, but not limited to, qualifications, experience, and equity. The full salary range for this position is $30.56 - $60.82 hourly. The budgeted salary range that the University reasonably expects to pay for this position is about $30.56 - $35.00 Hourly.
Qualifications
Required:
- Working knowledge of health education issues and programs including nutrition, lifestyle and behavioral issues.
- Bilingual English and Spanish language skills.
- Ability to establish and maintain cooperative working relationships, and work effectively as a member of an interdisciplinary team.
- Excellent communication skills, and the ability to effectively communicate with a wide variety of people of various educational and socio-economic backgrounds.
- Excellent organizational skills and proficiency in using Microsoft applications such as Word, Excel and Access.
- Ability to travel between clinics and community organizations to conduct health education programs, presentation, services and outreach.
- Ability to function efficiently and independently, and to plan and prioritize daily work activities to meet clinic goals and objectives.
Strongly Preferred:
- Bachelor's degree in Health (or a related field) with two years of experience providing health education services in a public health or primary health care organization OR seven years of experience with prenatal services.
