General Summary

The Community Dementia Program Manager is responsible for providing high-quality dementia-specific assessment and programming, as well as community education, resource acquisition, and communication to qualified members of the community. This role requires strong skills in assessment, program planning, and individualized treatment to promote the health and well-being of Senior Association and Club Memory members.



Essential Duties & Responsibilities


  • Interview and assess community members for dementia risk and appropriateness for Club Memory membership.



  • Assess psychosocial needs, coping mechanisms, family dynamics, strengths, and risk factors to meet education, care partner skill-building, and coping needs for members and their families.



  • Provide dementia-specific programming in accordance with Club Memory guidelines, including therapeutic activity sessions tailored to dementia populations (e.g., support groups, exercise, music, relaxation, stress reduction).



  • Develop, implement, and evaluate programs to meet the resource, clinical, and psychosocial needs of members and families.



  • Help members access resources for care plans and provide referrals to community organizations and agencies as needed.



  • Communicate effectively with the Sibley team regarding member and group needs.



  • Document attendance, communications, and changes in a timely and consistent manner.



  • Participate in and provide in-service education on dementia for community members, families, professionals, and staff.



  • Advocate on behalf of Club Memory members and their families.



  • Provide emotional support and education on available resources and the dementia disease process to members and families.



  • Collaborate closely with physicians and other healthcare professionals to enhance understanding of social and emotional factors affecting members' health and coping.



  • Build strong relationships with members, families, and staff.



  • Demonstrate strong communication skills, including public speaking, interviewing, and assessment.



  • Assist in writing grant applications.



  • Procure and maintain necessary supplies and equipment for activities.



  • Maintain professional growth and development by setting annual goals and expanding knowledge while keeping clinical expertise current.



  • Demonstrate strong diagnostic and report-writing skills.





Other Duties & Responsibilities


  • Conduct member community meetings and/or public trainings as required by program needs.



  • Perform other related duties as assigned.





Skills & Abilities Required


  • Physical ability to meet job demands, including sitting for long periods, manual dexterity for materials, and capacity to evaluate member activity.



  • Flexibility and positivity in stressful situations, demonstrating resourcefulness, independent reasoning, calm demeanor, and professionalism.



  • Ability to speak comfortably to large groups, communicate clearly in writing and verbally, and use therapeutic communication with members and care partners.



  • Strong organizational skills to set priorities, coordinate activities across departments, and adjust programming to member needs.



  • Ability to recognize and respond immediately to serious or emergency situations.



  • Skill in performing crisis intervention and providing counseling for members, families, and staff as needed.



  • Capacity to meet the unique needs of geriatric and adult members and their care partners to maximize functioning.





Minimum Education & Clinical Experience Required


  • Bachelor's Degree required.



  • Experience in Nursing, Social Work, Counseling, Family Therapy, or Recreation Therapy preferred.




Salary Range: Minimum 32.14/hour - Maximum 53.01/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
