Cencora, in partnership with Tidal Health Atlantic, is seeking a Full-Time Medication Therapy Management (MTM) Pharmacist to deliver pharmacist-led clinical services across our ambulatory care network as part of the AGHRx RediScripts Pharmacy team. This position is focused on improving patient outcomes, strengthening provider relationships, and advancing pharmacy-led value-based care.This is a unique opportunity to shape the future of pharmacist-led MTM services within a leading health system partnership. The MTM Pharmacist will be a key driver of clinical outcomes, patient engagement, and pharmacy innovation at Tidal Health Atlantic. This associate will engage directly with patients and providers to optimize therapy, identify clinical opportunities, and expand pharmacy utilization. This is an exciting opportunity for a pharmacist who thrives in a collaborative, provider-facing role with both clinical and strategic impact.

Location: Onsite- Berlin, Maryland

Responsibilities:

  • Provide comprehensive MTM services, including medication reviews, transitions of care, and adherence interventions.

  • Conduct purposeful provider rounds and maintain visibility in practice settings to strengthen referral pathways.

  • Document MTM encounters and coordinate follow-up with pharmacy and care teams.

  • Monitor performance metrics (KPIs, dashboards) and complete MTM claims/reimbursement opportunities.

  • Participate in health system committees, clinical initiatives, and community outreach events.

  • Build and expand provider and community partnerships to grow MTM services.

Education:

Normally requires a Pharm D degree from an accredited School of Pharmacy a Degree in Pharmacy from an accredited College of Pharmacy; current, unrestricted pharmacist license through the appropriate state board of pharmacy.

Normally requires zero (1) to three (3) years of directly related and progressively responsible retail/ambulatory care pharmacy experience. Prior experience in MTM, ambulatory care, or clinical pharmacy preferred.

Skills and Knowledge:

  • Familiarity with MTM platforms (Outcomes, PrescribeWellness, EQUIPP) is a plus.

  • Ability to communicate effectively both orally and in writing, provider engagement and documentation skills.

  • Ability to work quickly and accurately under time and volume constraints

  • Strong leadership skills

  • Strong interpersonal skills

  • Ability to resolve patient issues quickly and creatively in order to improve customer satisfaction

  • Familiarity with pharmaceutical products, services and drug interactions

  • Strong organizational skills; attention to detail

  • Good analytical skills

  • Knowledge of Microsoft Word, Excel, PowerPoint and Outlook

*This Salary Range reflects a National Average for this job. The actual range may vary based on your locale. Ranges in Colorado/California/Washington/New York/Hawaii/Vermont/Minnesota/Massachusetts/Illinois State-specific locations may be up to 10% lower than the minimum salary range, and 12% higher than the maximum salary range.
