PCA Field Supervisor

The Field Supervisor is responsible to meet with the HCBS Participant and Attendant in their home to evaluate and assess the needs of the Participant. The Field Supervisor is to schedule timely follow-up visits to ensure the delivery of tasks assigned to the Participant care plan. They will complete all necessary paperwork to meet regulatory and company requirements and communicate pertinent information to the Branch Manager and administrative staff.

Complete supervisory visits per company/contract specifications
• Complete all necessary documentation for supervisory visits to meet regulatory and company requirements
• Assist the Participant with any issues or questions relayed during the home visit
• Report issues, concerns, and problems to Branch Manager and administrative staff.
• Enter home visit information in the correct systems
• Document and track all home visit contact information with the Participant
• Support Administrative staff when requested by Supervisor
• Cooperate in external/internal investigations
• Supervise and evaluate staff
• Monitor staff, identify problem situations, and develop/implement solutions for optimum outcomes
• Evaluate job performance of Attendants and conduct performance reviews
• Responsibilities may include assigning tasks and providing supervision to other Branch or Administrative Employees

• Excellent customer service and communication skills; must be able to function well under pressure, make decisions quickly, and manage a large amount of information
• Direct care and supervisory experience working with vulnerable populations

• Solid experience working with the public, presenting a friendly, professional appearance while interacting with people
• Ability to communicate both verbal and written

• Available after office hours on-call, and weekends
• Ability to service participants when attendant is not available

REQUIRED:

• Have at least two (2) years of supervisory experience in programs dealing with elderly and disabled individuals
• AND Satisfy at least one of the following:
• (1) Have a bachelor's degree in social work, home economics, or a related profession with two (2) years of direct experience working with aged and disabled persons, or
• 2) A licensed RN or Licensed Practical Nurse (LPN) with two (2) years of direct experience working with aged and disabled persons, or
• (3) Be a high school graduate with diploma or have a General Educational Development (GED) certificate, and four (4) years of direct experience working with aged and disabled persons.
