9am-5pm

The ideal candidate will possess the following qualifications:

• A valid Driver's license and reliable vehicle: This position requires travel to various schools, homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
• Must be able to work 9am -5pm Monday-Friday

Educational Requirements

• A Bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education

or

• A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years' experience in direct contact with mental health consumers.

We are seeking a Community-Based Behavioral Health Case Manager to join our Program in Warminster, PA, serving Clients in Lower Bucks County.

The Community-Based Behavioral Health Case Manager is responsible for providing comprehensive case management services to individuals and families in the community. This includes assessing client needs, developing care plans, connecting clients with resources, and advocating on their behalf. The goal is to support client stability, independence, and improved quality of life through individualized, culturally sensitive, and strengths-based services.

The Behavioral Health Case Manager will:

• Conduct in-home or community-based visits.
• Develop and implement individualized service plans in collaboration with clients.
• Provide referrals and linkage to medical, mental health, housing, employment, education, substance use, and social support services.
• Monitor client progress through regular follow-ups, home visits, and coordination with service providers.
• Advocate for client needs within various systems including healthcare, housing, education, and public benefits.
• Maintain accurate, up-to-date documentation in compliance with agency, state, and federal standards.
• Facilitate crisis intervention and support clients in navigating emergencies or acute needs
• Attend agency meetings, community meetings, and trainings as required.
• Build and maintain strong partnerships with community agencies and service providers

Earn $21.75 per hour OR $23.75 per hour with the selection of our Enhanced Pay option!
