Overview

The Oncology Patient Care Navigator positively affects patient care by facilitating the multidisciplinary clinical system. The Oncology Patient Navigator improves patient preparedness for treatment via education and psychosocial support, by facilitating interaction between patients and their physician(s), providing logistical support and by facilitating the referral process. The Oncology Patient Navigator assists in the development of apporpriate education materials and program monitoring to promote coordination of care and patient compliance/satisfaction.

Responsibilities

  • 1. Provides support to patient and care giver(s) though their cancer journey through support, guidance and direction including both inpatient and outpatient care settings.
    • 1.1 Assists patients in understanding their diagnosis, treament options and the resources available including educating patients about clinical trials and technologies.
  • 2. Support fund raising activities.
    • 2.1 Build a community presence to assist the Cancer program with development of community awareness events and participation in fund-raising efforts.
  • 3. Participates in COE's and case reviews for specific tumor types.
  • 4. Assists with provision of community education and screening activities.
  • 5. Represents the Smilow Cancer Hospital at Bridgeport as a high level professional at all times.
  • 6. Develops measures for success and assists with tracking data and reporting.
    • 6.1 Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic and Cancer Committee goals.
  • 7. Facilitate appointments for consults and support services within established service standards, including social work, dieticians, physicians, etc.
  • 8. Coordinate multidisciplinary planning conferences and present NCCN guidelines for discussion and incorporate into treatment planning.
  • 9. Participates in department specific quality improvement activity relevant to program and role.
  • 10. Adhere to established productivity measures.
  • 11. Develops and maintains positive working relationships with community agencies, facilities, and services providers to enhance the availability of resources and to facilitate patient access.
    • 11.1 Responsible for outreach efforts and to establish and maintain positive working relationships with key customers (physicians, radiology, nurses, social services, etc.).

Qualifications

EDUCATION

BSN or MSW required.

EXPERIENCE

3-5 years oncology clinical experience. Requires independent problem solving, critical thinking and decision making

LICENSURE

CT RN license or MSW. ONS certification desirable.

SPECIAL SKILLS

Excellent communication skills and clinical expertise. Good organizational skills.

PHYSICAL DEMAND

Must be able to stand, sit and walk through out the day. Drive to outside meetings and appts. Flexible to attend evening and weekend functions periodically
