Located in Greenwood, IN, our 132-bed facility provides a peaceful setting for those seeking treatment for mental illness and chemical dependency. Position Summary: FT Days

This employee is responsible for the discharge planning for patients receiving care on the acute mental health units. The Discharge Planner provides patients with a review of treatment options beyond Valle Vista Health System. The Discharge Planner represents VVHS in coordination of treatment to follow up for patients within the community. The Discharge Planner works closely with other members of the social services team from the time of intake to plan an appropriate discharge plan.

Job Duties/Responsibilities:

  • Work directly with patients and their families to ensure patients have appropriate safe plans after discharging from acute behavioral health hospital
  • Coordinate aftercare services with community providers.

REQUIREMENTS:

Education: Bachelor's degree in social work or other social science is required

Experience: Six months to one year of experience working with a psychiatric in-patient population preferred.

Additional Requirements: Must be at least 21 years of age.

Work Hours: 8:30 am - 5:00 pm Monday - Friday. Must be able to work periodic weekends
