Responsibilities

Clinical Care

  • Provide comprehensive outpatient evaluation, assessment, diagnosis, and management of patients with urogynecology and pelvic floor disorders.
  • Obtain detailed medical histories, perform physical examinations, and develop individualized treatment plans in collaboration with supervising physicians.
  • Order, interpret, and communicate diagnostic testing, laboratory results, and imaging studies.
  • Prescribe medications and therapies in accordance with state regulations, organizational policies, and clinical practice guidelines.
  • Perform office-based procedures within scope of practice and clinical competency.
  • Participate in patient triage, follow-up care, care coordination, and management of acute and chronic urogynecology conditions.
  • Collaborate closely with physicians, nurses, medical assistants, and other members of the interdisciplinary care team to ensure high-quality patient outcomes.
  • Support care delivery across multiple clinic locations throughout Lower Fairfield County, as needed.

Patient Education & Support

  • Educate patients and families regarding diagnoses, treatment options, medications, procedures, and postoperative care.
  • Provide counseling and support related to pelvic health conditions, bladder disorders, pelvic organ prolapse, urinary incontinence, and other urogynecology concerns.
  • Empower patients to participate in their care through education on preventive health measures and available community resources.
  • Deliver compassionate psychosocial support while maintaining patient dignity and confidentiality.

Documentation & Information Management

  • Maintain accurate, timely, and thorough documentation in the electronic medical record (EMR).
  • Ensure compliance with organizational policies, regulatory requirements, and professional standards.
  • Protect patient confidentiality and safeguard sensitive information in all forms of communication and documentation.

Quality & Patient Safety

  • Demonstrate professionalism, ethical practice, and a patient-centered approach to care.
  • Participate in quality improvement initiatives, patient safety programs, and departmental performance improvement activities.
  • Adhere to evidence-based practice standards and organizational policies to ensure the delivery of high-quality care.
  • Identify and report patient safety concerns, near misses, and opportunities for process improvement.

Professional Development

  • Maintain licensure, certifications, and continuing education requirements.
  • Stay current with advancements in urogynecology, women's health, and advanced practice clinical guidelines.
  • Serve as a resource and mentor for students, new providers, and clinical staff as appropriate.
  • Participate in research, quality initiatives, protocol development, and educational programs that support departmental and organizational goals.

Qualifications

Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.

Experience

  • Clinical working experience as an NP/PA in the specialty/service line is preferred but not required.
  • APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS hospital and go through the Medical Staff credentialing process.
  • To be eligible, evidence of current competence to practice as an APRN, PA, CRNA, or CNM must be provided.
  • This includes obtaining appropriate reference letters from physicians and other practitioners during the Medical Staff appointment process.

Licensure

  • Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certified, when indicated for position profile DEA License.
  • Pediatric position profile PALS, when indicated for neonatal care NRP Certification.
  • Additional licensure, certifications and/or continuing education credits may be required during employment based on position and area of specialty. OR Connecticut State Registered Nurse License, Connecticut State Nurse Practitioner License, Board specialty certification, as appropriate for position profile ACLS/BLS Certified, when indicated for position profile DEA License.
