Job Summary:

The RN Care at Home brings the essential elements of inpatient-acute and ambulatory care into the patient's home, delivering both hands-on episodic care and remote, technology-enabled monitoring through a single, continuous model of clinical care coordination. The RN Care at Home provides quality care across the full continuum of the Care at Home program - Hospital at Home and Remote Monitoring - applying the nursing process whether care is delivered in person in the patient's home or remotely through monitored review of biometric and symptom data. The RN moves fluidly between acute, in-home episodes of care and ambulatory monitoring of patients, accesses and documents care for patients across both programs. The RN leads care coordination across a multidisciplinary healthcare team- including Mobile Integrated Health (MIH), Patient Experience Navigators (PENs), and Telehealth Coordinators - driving therapeutic interventions, aligning logistics, and ensuring a seamless, high-quality and safe patient experience across care settings.

Position Responsibilities:

Perform and document the nursing process - assessment, diagnosis, planning, implementation, and evaluation - for patients receiving acute, in-home episodic care (Hospital at Home) and for patients enrolled in ambulatory remote monitoring, whether care is delivered in person in the patient's home or through periodic, trend-based review of remote biometric and symptom data. Perform intake and ongoing assessments, track and trend clinical data, review and follow up on patient education, implemented care plans, and coordinated services. Triage, identify, and act on changes in patient condition and escalating care needs, supporting transition of patients to the most clinically appropriate level of care.

Utilize and instruct patients and families on the use of the telehealth tablet and remote patient monitoring (RPM) equipment used for biometric data capture, patient symptom reporting, and telehealth visits, and educate patients on management of chronic conditions.

Serve as the primary care coordinator, integrating multidisciplinary efforts across the Care at Home continuum: collaborate and communicate extensively with Mobile Integrated Health (MIH) on home visit findings and escalation, with Patient Experience Navigators (PENs) on patient onboarding, scripting, and experience issues, and with Tele Health Coordinators on visit scheduling, technology troubleshooting, and equipment logistics.

Adheres to policies and procedures of the organization, including HIPAA and Patient confidential regulations.

Maintains nursing supplies and anticipates and restocks needed supplies. Maintains safe and clean working environment.

Position Qualifications Required:

Required Experience:

Minimum of 2-3 years recent acute care nursing experience within the last 5 years in settings such as Med/Surg, ED, ICU/CCU or comparable unit. Excellent assessment skills and clinical judgement, with the ability to function independently and manage complex care across both episodic and remote monitoring care models.

Required Education:

Graduate of an accredited nursing program.

BSN preferred.

Training / Certification / Licensure:

Current State of New Jersey nursing license to practice as a registered nurse. Current CPR certification and BLS.

Must have current proof of valid driver's license and auto insurance. Required for staff rotating into in-home visit assignments
