Location: Lourdes Health System - 1600 Haddon Ave Employment Type: Employee Employment Classification: Regular Time Type: Full time Work Shift: 1st Shift (United States of America) Total Weekly Hours: 40 Additional Locations: Job Information: Summary: Provides pharmaceutical care according to professional standards and practices and complies with all Federal and State regulations in the practice of Pharmacy at Virtua Our Lady of Lourdes. Critical Care clinical pharmacy services to include direct patient care at Virtua Our Lady of Lourdes which is the center of the health system for the following: Cardiac surgery (including LVAD), abdominal transplant, and neuro / neurosurgery. Rounding will include rotation in MICU, Cardiac Surgery ICU/CCU, and Neuro ICU. This position is the third ICU Clinical Pharmacy position, which allows for one clinical pharmacy specialist in each ICU. Additional responsibilities include working on system-wide guidelines and protocol standardization and education for pharmacy department and clinical departments as needed. Participation in system-wide committees such as Critical Care Committee and Pharmacy & Therapeutics Committee, in addition to precepting pharmacy residents and pharmacy students. Position Responsibilities: * Participates in patient rounds with appropriate individuals including physicians, nurses and other health care providers. * Evaluates medication prescribing patterns and identifies opportunities for improving drug therapy. * Drug Use Evaluations * Provide education to patients/families, and other health care practitioners. * Participate in medication safety initiatives * Works within key sub-specialty areas (ex. Critical Care and sub-specialties) to develop and maintain specific routine monitoring criteria for these patients. * Represents the Pharmacy Dept. in system wide and divisional committees as needed. Required Education: Graduate of an accredited College of Pharmacy with Doctor of Pharmacy (PharmD) degree or BS and comparable clinical experience. PGY-2 in Critical Care Preferred Training/Certifications/Licensure: Active New Jersey pharmacy license.
